Payment Due, 24 Hour Cancellation And “No Show” Fee Policy
We pledge to do our very best to care for your pet’s needs. In return we ask you to accept financial responsibility for the charges incurred in the treatment of your pet. Payment is due at the time services are rendered. I understand that should I be delinquent; my information will be released to a third party collection agency to assist with collecting fees associated with the treatment of your pet. Recognizing that everyone’s time is valuable, and the appointment time is limited, we ask that you please provide a 24-hour notice if you are unable to keep your appointment. Each time a patient misses an appointment without you as owner providing us with proper notice, another patient is prevented from receiving care. Therefore, Family Pet Clinic reserves the right to charge a fee of $65.00 for each missed (No Show/ Cancelled) appointment not cancelled within a 24-hour advance notice.
“No Show” fees will be billed to your account and must be paid PRIOR to your next appointment.
